Social services are provided at each facility to assist residents in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs. As part of the multidisciplinary team, the social worker conducts a routine, comprehensive assessment of the psychosocial needs of each resident and makes referrals for needed clinical or support services as warranted. The social worker is a key facilitator of mental health services. Social services also include assistance with end-of-life treatment decisions and advance care planning, financial and legal matters, dental, vision and hearing referrals as ordered, discharge planning, community resource planning, making arrangements for the provision of needed clothing and personal items, etc.
